Prescription Refills - PLEASE BE SURE TO COMPLETE THE ENTIRE ORDER FORM ***PLEASE CHECK THAT IT WAS SUBMITTED*** In our ongoing effort to make your pet's health care as convenient and easy as possible, you can now request a refill for your pet's prescription by submitting the following form. Please be sure to fill in ALL the requested information. All prescription refills must be approved by Dr. Donna and are only available to our current clientele. Online orders are checked Monday, Tuesday, Thursday and Friday, twice a day, 10:00am and 2:00pm. Dr. Donna is off on Wednesdays and not all prescriptions will be available for refill on that day. All prescriptions requested on Saturdays or Sundays will not be filled until Monday. We will notify you via email when your pet's prescription is approved and ready to be picked up. If you would prefer to have the prescription mailed to you, once approved, we will call and inform you of the total cost of the prescription. (Including shipping charges and delivery confirmation - minimum charge $9.00) We will also request a credit card number by phone at that time. Please make sure to fill out the shipping address information to ensure proper delivery of your prescription.
